Mihaylov made his name on the pitch with the great Levski Sofia side of the 1980s, winning the domestic league and Bulgarian Cup three times each as he followed in the footsteps of his father Biset, who had enjoyed a similarly successful career for both club and country.

Mihaylov also played for Belenenses (Portugal), Mulhouse (France), Reading (England), Botev Plovdiv and Slavia Sofia (Bulgaria) and Zurich FC (Switzerland) during his club career, but his finest hour came as captain of Bulgaria, who he led to the 1994 FIFA World Cup semi-finals, saving three penalties in a round of 16 shootout against Mexico.

Having retired from playing in 1998, Mihaylov turned to administration and in 2001 became the BFU's first vice-president, eventually serving as president between 2005 and 2023. In 2011, he was elected to the UEFA Executive Committee, serving two terms.

"With the utmost sadness, we announce the passing of legendary goalkeeper and former BFU president Borislav Mihaylov;" said a statement from the federation. "He devoted his life for the development of the game. We pass our condolences to his family and friends. Rest in peace."

Former team-mate and fellow Bulgarian great, Hristo Stoichkov, added: "Why so early? Our captain, our leader, our friend has left us. Tears cannot be stopped. You were a great man and a great player. So sad. I embrace you, my friend. Rest in peace."

Mihaylov's son, Nikolay, continued the family dynasty between the posts, becoming a Bulgaria regular following his international debut in 2006. Reacting to his father's passing, he said: "Dad, I miss you. There are no words to describe the pain. This morning I lost not only my father, but my hero and my inspiration. Thank you for every embrace, for every lesson and advice, for your endless love. Rest in peace, dad. You will live in every one of us."
